2018 Charge Conference Forms


All Charge Conference forms are to be downloaded to your computer, saved in your computer, completed (don’t forget to save again!). For submission of forms, your District Office will provide instructions on the preferred method. The forms are designed to be completed on a computer and the answer boxes will expand to accommodate the length of your response.

This is the “off year” for submitting the “Safety and Accessibility Inspection” form. The Action Plan does not have to be submitted.

All Safe Sanctuary policies should be updated according to the Resolution 2016-09 (Safe Sanctuaries Resolution) [PDF].

The Accountability Agreement between the EPA UMC and the BSA was introduced in 2016 and must be completed every three years. Therefore this must only be completed this year:

  • if it was not completed in 2016
  • if there is a new BSA troop at your church
  • if there has been a change in scout leadership

DUE DATES:  Your clergy profile is due 2 weeks prior to your pastoral interview.  All other forms except the Church Budget and Fund Balance report are due two weeks before your charge conference or by Nov. 15 – whichever comes first.  Pay close attention to the 2018 Church Forms Schedule and plan ahead. Do not make the DRA have to chase down your reports!

We hope that you will find the annual reporting process to be an opportunity to celebrate your church’s achievements, a useful tool to evaluate your church’s ministry goals and objectives, and a helpful process to ask important accountability questions. All forms are downloadable Word Documents, except where noted otherwise.

Due 2 weeks prior to your pastoral interview:
Due 2 weeks before your charge conference, or by Nov. 15 (whichever comes first):
  • 2018 Fund Balance Report–This report is to be completed between Jan. 1 and Feb. 1. This report should not be included with your other charge conference forms.
    • Submit a copy of the Summary Page form (Complete with all signatures) to your District Office 2 weeks prior to your Charge Conference, but no later than November 30th, 2018.
    • Submit a copy of the UMPIP Contribution Election form to the EPAUMC Benefits Office no later than December 19, 2018. Failure to submit may mean a delay in correct billing for 2019.
